Evaluation of the treatment outcomes of benign paroxysmal positional vertigo using the Epley maneuver at Thai Nguyen University of medicine and pharmacy hospital

Abstract

Objectives: To describe the clinical characteristics and evaluate treatment outcomes of benign paroxysmal positional vertigo (BPPV) treated with the Epley maneuver at Thai Nguyen University of Medicine and Pharmacy Hospital. Methods: A cross-sectional descriptive study was conducted on 30 patients diagnosed with posterior semicircular canal BPPV at the Department of Internal Medicine, Thai Nguyen University of Medicine and Pharmacy Hospital. Diagnosis was confirmed using the Dix-Hallpike test. All patients were treated with the Epley maneuver. Treatment outcomes were assessed immediately after the intervention and after 24 hours. Results: The study population consisted mainly of middle-aged and elderly patients, with those aged over 60 years accounting for the highest proportion (36.6%). Females predominated, representing 66.7% of cases. Rotational vertigo was present in all patients, while imbalance was reported in 86.7% of patients and nausea in 66.7%. Right-sided involvement was observed in 60.0% of patients. After the Epley maneuver, good outcomes were achieved in 53.3% of patients, fair outcomes in 26.7%, and poor outcomes in 20.0%. Before treatment, 100% of patients had rotational vertigo and a positive Dix-Hallpike test; after treatment, this proportion decreased significantly to 20.0%. The difference was statistically significant (p < 0.001). Adverse effects were mild and transient, with no serious complications reported. Conclusions: The Epley maneuver is an effective and safe treatment for posterior semicircular canal BPPV and should be widely applied in clinical practice.
