Overcoming Pain Treatment Challenges through Internal Medicine Clinical Skills

Abstract

Background: According to the 2015 survey conducted by the Hanoi Pain Society, the prevalence of pain in Vietnam was remarkably high, affecting 86.53% of the population. Among these individuals, 63.43% experienced chronic pain that interfered with occupational activities (67.71%), while 37.84% reported severe or very severe pain. With a mean age of 54.1 years, patients had endured pain for an average of 1.86 years. Pain management also imposed a considerable economic burden, with 46.55% of patients spending ≥ USD 150 on medical consultations and treatment within the preceding six months. Despite these expenditures, treatment outcomes remained suboptimal: only 61.98% of patients were satisfied with pain management, whereas 19.5% reported dissatisfaction. Objectives: To identify factors limiting the effectiveness of pain management, propose practical strategies to improve clinical outcomes, and present five complex cases successfully managed to highlight key clinical lessons. Subjects and Methods: This study analyzed clinical activities and treatment outcomes from 10 pain management units affiliated with the Hanoi Pain Society. In addition, expert panel discussions were conducted to share clinical experience and optimize pain management strategies. Results: Multiple barriers to effective pain management were identified. Objective factors included delayed healthcare-seeking behavior leading to advanced and chronic disease, the absence of objective paraclinical tests specifically confirming pain, limited recognition of pain as a healthcare priority, and inadequate legal and policy support for pain management services. Subjective factors included insufficient formal training in pain medicine, inadequate clinical skills in pain assessment, diagnostic difficulties due to overlapping pain generators within the same anatomical region, the complexity of selecting appropriate treatment modalities among numerous pain syndromes, excessive reliance on clinical guidelines, and reduced confidence among young physicians when managing patients previously treated by multiple specialists. Strategies to improve treatment outcomes include comprehensive education and continuous professional training in pain medicine; a systematic approach to pain assessment based on thorough anatomical knowledge, neurological and musculoskeletal examination skills, and a solid understanding of pain mechanisms; meticulous history taking and physical examination; scientific analysis to accurately identify pain-generating structures; and flexible, individualized application of pharmacological and non-pharmacological therapies. Mastery of interventional pain procedures, including nerve blocks and musculoskeletal injections (tendon, muscle, and joint injections), is also essential. Analysis of five successfully treated complex cases demonstrated that proficiency in interventional techniques, combined with sound clinical judgment, individualized therapeutic decision-making, confidence, and decisive clinical practice, are key determinants of successful pain management. Conclusion: Improving pain management outcomes requires both systematic training in pain medicine and the development of strong clinical reasoning skills. Accurate diagnosis, individualized treatment strategies, and competence in interventional pain procedures are fundamental to achieving optimal clinical outcomes, particularly in patients with complex or refractory pain syndromes.
